Windows 10 After attempted fixes and 2 months of smooth running my PC is back to freezing again when gaming.

So to summarise, I've sought out help from this forum before and it seems I just can't get my pc fixed.I hate to continue pestering for help but I really don't know what to do.It seems it may be to do with my Graphics card possibly? I've ran this game absolutely fine for 2 months straight (after attempted fixes. The crashes before the 'attempted fixes' were the exact same as this one I had tonight.) until I'm back to the game & PC freezing and restarting again.I've included the Minidmp at the bottom of this post, if this is even a thing; I'd like to do the complete 9 yards sweep to check/f
