Windows 11 Games Crash on my computer and the D: drive pops up, how do i solve this?

Hello, When playing games (Skyrim, Starfield), the game will just down. The PC makes a noise as if a drive has just been detected and the explorer opens showing me the contents of D: drive. This happens often but not after a set amount of time. I've included some of the info from dxdiag to help diagnose this.
