Windows 10 Hardware Error - LiveKernelEvent 141 - I don't believe its being caused by my GPU and I need help

I've been dealing with this issue for ages. I can't figure it out anymore. I had determined the GPU was the issue after changing GPU's made it stop for weeks. But its back. It has been the bane of my existence for almost 2 years now and it causes various games or programs to crash whenever it pokes its head.
