Windows 10 Microsoft Print to PDF - the document is different from what I see on the monitor

Trying to print a Medicare claim to a pdf which I've done hundreds of times over the years. Recently, what I see on screen is not how it is displayed on both the print window and on the final document itself. I've tried Save as PDF, Nitro Pro 8, Microsoft Print to PDF. Using Windows 10 Home. See screenshots below.
