When the mouse cursor changes to one that should be displayed with an offset (e.g., the hand cursor should be displayed slightly to the left so that the tip of the finger matches the mouse position) it is not and instead is drawn with the same position as the default pointer.This is not specific to any program. Hovering a link the web browser shows the hand cursor to the right of where it should be. The cross-hair pointer in paint is down and to the right of where it should be.
